Program Overview
Through the Family Managed Home Care program, eligible clients or their substitute decision makers receive funding to purchase home care services or employ care providers. This offers greater flexibility and choice as those approved for Family Managed Home Care choose their provider, determine how and when the care plan is delivered, and handle all related administrative tasks, including managing providers, funding, and reporting requirements.
Assessment:
Clients must first have an assessment completed by Ontario Health atHome to determine their needs, meet the general and specific eligibility requirements for traditional home care, and have a plan of service developed.

Real-Time Updates: Access daily care notes, progress reports, and real-time updates from our nurses and personal support workers, ensuring you always know how your loved one is doing.
Medication & Treatment Tracking: See medication schedules, upcoming treatments, and any changes to your loved one’s care plan, all updated in real time to prevent miscommunication.
Care Team Communication: A direct line to care team members, so you can ask questions, share concerns, or provide feedback without the need for constant phone calls or emails.
Scheduling Transparency: View upcoming appointments, home visits, and shift schedules, helping you coordinate with family members and care team members efficiently.
Secure Document Sharing: Access important medical documents, care plans, and assessments all in one place, ensuring family members and authorized healthcare providers are on the same page.
Emergency Alerts & Change Notifications: Get notified of any significant health updates, emergency situations, or changes in care plans, giving you the ability to respond quickly when needed.
When Ontario Health atHome makes a decision on whether a client’s situation qualifies as an extraordinary circumstance, the following factors are considered:
The nature, frequency, and intensity of care required
Unique scheduling issues
Language or communication barriers
Cultural considerations
Lack of Ontario Health atHome contracted service providers
Rural or remote location of care
